Application for Health Insurance Card
一、New Students Enrollment
（一）When registering, overseas and Mainland Chinese students can obtain the application form for the health insurance card at the service counter of the Overseas Students Advising Division
（二）The application form should be submitted to the Overseas Students Advising Division before September 30th. (Those who already have a health insurance card do not need to submit the application, and their current card can continue to be used.)
（三）The Overseas Students Advising Division will coordinate with the National Immigration Agency to proactively enroll students who have been residing in Taiwan for 6 months or more in the National Health Insurance Program.
（四）The Overseas Students Advising Division will submit the qualified students' application forms to the National Health Insurance Administration for processing the health insurance card. After receiving the card, the office will notify students via email to collect their cards.
二、Replacement of Lost or Damaged Health Insurance Card
Please apply for a replacement health insurance card in person with your Alien Resident Certificate (or Taiwan ID card), and pay a handling fee of NT$200, through one of the following methods:
（一）Apply in person at the National Health Insurance
Administration service center. The card can be received on the same day, but may require a waiting time of 15-30 minutes depending on the situation.
（二）Apply at a designated post office by submitting the health insurance card application form. The card will be mailed to the designated address in about 5-7 working days.
三、Replacement of Health Insurance Card due to Personal Information Changes
For those who need to apply for a replacement health insurance card due to changes in name, Taiwan ID number, Alien Resident Certificate unified number, etc., please apply in person with your Alien Resident Certificate (or Taiwan ID card), and pay a handling fee of NT$200, through one of the following methods:
（一）Apply in person at the National Health Insurance
Administration service center. The card cannot be received on the same day, and applicants should inquire about the card pick-up time by mail or in person at the service counter.
（二）Apply at a designated post office by submitting the health insurance card application form. The card will be mailed to the designated address in about 5-7 working days.
