Application for Health Insurance Card
（一）After receiving the Alien Resident Certificate (ARC)/Taiwan Area Resident Certificate, new students are required to fill out the NHI Card Application Form for Overseas Chinese Students by September 30 (those who are dependent on others for NHI or already have an NHI card do not need to fill out this form).
（二）Overseas Chinese and Mainland Chinese Students Advising Division will proactively enroll students who have resided in Taiwan for six months in the National Health Insurance (NHI) program, based on immigration data provided by the National Immigration Agency.
（三）Overseas Chinese and Mainland Chinese Students Advising Division will apply for NHI cards from the National Health Insurance Administration. Once the cards are received, students will be notified via email with pick-up instructions.
二、Replacement of Lost or Damaged Health Insurance Card
Please apply for a replacement health insurance card in person with your Alien Resident Certificate (or Taiwan ID card), and pay a handling fee of NT$200, through one of the following methods:
（一）Apply in person at the National Health Insurance
Administration service center. The card can be received on the same day, but may require a waiting time of 15-30 minutes depending on the situation.
（二）Apply at a designated post office by submitting the health insurance card application form. The card will be mailed to the designated address in about 5-7 working days.
三、Replacement of Health Insurance Card due to Personal Information Changes
For those who need to apply for a replacement health insurance card due to changes in name, Taiwan ID number, Alien Resident Certificate unified number, etc., please apply in person with your Alien Resident Certificate (or Taiwan ID card), and pay a handling fee of NT$200, through one of the following methods:
（一）Apply in person at the National Health Insurance
Administration service center. The card cannot be received on the same day, and applicants should inquire about the card pick-up time by mail or in person at the service counter.
（二）Apply at a designated post office by submitting the health insurance card application form. The card will be mailed to the designated address in about 5-7 working days.
